In your Outlook Inbox, right click the message you would like to retrieve header information from and select Options:

In the Message Option box that appears, you will find header information for the e-mail at the bottom of the box.  Highlight all of the information in the Internet Headers box, copy it and paste it into the form at the bottom of this page.
